The Fall season is almost upon us and hopefully we will be back to normal! The summer season officially switches to the Fall on Monday, August 7th (Football) & Monday, August 21st (All other sports), so this is a new season (technically) and we will need all of the medical paperwork submitted in order to be cleared for those practices. Keep in mind even if you were cleared for summer practices, you will need to be cleared again for this fall season. Oftentimes there are errors or omissions with paperwork. Submitting paperwork by Thursday August 3rd, is very strongly recommended so you are certain to be cleared for the first day of practice. As always, there are four items needed in order to be cleared for participation for the Fall season. The following links contain the files and information required for participation. These forms must be completed in full, and without errors or they will be returned for completion.
A physical, completed by your physician dated August 7, 2023 for Football/ August 21, 2023 for all other sports or more recent.
A health history update
“WHS Athletic Forms/Extra-Curricular Activities” to be electronically signed on the parent portal of Genesis. (Note: This only needs to be completed once per school year)
“Random Drug Testing” to be electronically signed on the parent portal of Genesis. (Note: This only needs to be completed once per school year)
A few notes:
-The physical must be completed in full and signed by the student, parent and physician in all the applicable places. Please take care to note before leaving the doctor's office that all values (height, weight, vision, etc.) must be filled in or the physical will be returned for completion.
-If the athlete sees an eye doctor, the physician who performs the physical might not perform a vision exam. In this case, a copy of an eye glass or contacts prescription must be provided.
-All athletes participating in a fall sport need to fill out a Health History Update (Item 2). While this is exactly the same form that was filled out for summer practices, the state considers the summer and fall separate seasons. Therefore, an additional HHU needs to be filled out for the fall season, as it does for winter and spring.
